Client Benefit Worksheet
This Worksheet is a Word document tool that will allow
you to provide your clients with all of their appointment information
in
one
place,
and allow you to store a copy of the information you give them on your
computer. During or shortly after your meeting with your client, you
can enter in important information about him/her, benefits he/she may
be eligible for, and appointment information, and any instructions for
him/her to follow for the appointment. Once you have filled in all of
the relevant and available information, you should give a copy of the
Worksheet to your client to use and keep a copy for your files.
Here are some tips for using
the Client Benefit Worksheet:
- Save the “Client Worksheet” file
on your desktop and create a different document for each of your
clients.
- Explain to your client how the information is laid out
and how they should use the Worksheet when you give it to him/her
the first time.
- Include your name and the agency for which you work in
case your client has more than one case manager or outreach worker.
- Copy
the “Appointment Schedule” table and "paste" it for
each one of your client’s appointments to ensure that
all of his/her appointment information is on one page.
- Tell your client to show the Client Benefit Worksheet to the person
they meet with so they can get your contact information in case they
have any questions or need to reach your client at a later date.
- Ask your client to have the “Appointment
Checklist” filled
out by the person with whom they are meeting and bring back a copy
for your files.
- Follow up
with your client after his/her appointment and add any
notes or action items to his/her Worksheet resulting from your
client’s
appointments.
